The Children’s Community Nursing Team is made up of a variety of health
care professionals. The team ensure that they provide high quality and safe
care enabling children and young people to remain in their home environment
whilst still getting the care they need. The team consists of:
Community Children’s Nursing Team Leader – this is a qualified nurse who
oversees the management and delivery of the service.
Community Children’s Nurses – these are qualified nurses who deliver
nursing care within the community setting. One of our nurses can prescribe a
limited range of medicines.
Specialist Care Co-Ordinator (formerly known as Continuing Health Care
Co-Ordinator) – this is a qualified nurse who co-ordinates the care
provision for children with long-term, multiple and complex health needs.
Health Care Support Workers – these are specifically trained Health Care
Support workers who deliver a variety of care in various locations, supporting
both children and young adults and their families to be cared for in their home
environment. The Health Care Support Workers also provide support to the
Community Children’s Nursing Team and liaise with the hospital to ensure that
specialist care can be provided within the community.
Specialist Nurses We have specialist nurses for asthma,
diabetes and epilepsy
Special School Nurses – these are qualified nurses who provide
specialist nursing care to a variety of children and young people with complex
health needs to ensure that they can successfully access education.
All of these health care professionals are supported closely by Consultant
Colleagues at Rotherham Hospital and also by Specialist Consultants at other
hospitals.
